โรคพยาธิปากขอเป็นโรคที่เกิดจากการติดเชื้อพยาธิปากขอ*ที่อยู่ตามพื้นดิน
โรคนี้พบได้ทุกภาคของประเทศ แต่จะพบในภาคใต้มากกว่าภาคอื่น ๆ มักพบในชาวไร่ ชาวนา ชาวสวน และเด็ก ๆ ที่เดินเท้าเปล่าบนพื้นดิน หรือในเด็กที่ชอบเล่นคลุกดิน
พยาธิปากขอ (Ancylostoma duodenale และ Necator americanus) มีขนาดยาวประมาณ 1 ซม. เกาะอาศัยอยู่บนผนังลำไส้ และดูดเลือดจากบริเวณนั้น ไข่พยาธิจะหลุดออกมากับอุจจาระ ซึ่งจะเจริญเติบโตบนพื้นดินที่ชื้นและมีความอุ่น พยาธิตัวอ่อนที่ฟักตัวบนดินจะไชเข้าทางผิวหนังของคนที่เดินผ่านไปมาหรือเด็กที่เล่นคลุกดิน เข้าไปในกระแสเลือด ไปยังหัวใจและปอด จากปอดพยาธิจะเคลื่อนตัวขึ้นมาที่หลอดลมจนถึงคอหอย แล้วจะถูกกลืนลงหลอดอาหารเข้าไปในกระเพาะและลำไส้เล็ก แล้วเจริญเป็นตัวแก่ต่อไปในลำไส้เล็ก นอกจากนี้ ถ้ากินอาหารหรือดื่มน้ำที่ปนเปื้อนพยาธิตัวอ่อนระยะติดต่อ พยาธิอาจไชผ่านเยื่อบุในปากและเข้ากระแสเลือดได้เช่นกัน
สาเหตุ
การติดต่อของโรคนี้ เกิดจากพยาธิปากขอตัวอ่อนที่อยู่บนพื้นไชเข้าเท้าของผู้ที่เดินเท้าเปล่า หรือเกิดจากกินพยาธิตัวอ่อนระยะติดต่อที่ปนเปื้อนอาหารหรือน้ำดื่ม
อาการ
เมื่อพยาธิไชเข้าเท้า อาจทำให้มีตุ่มแดงคันที่ผิวหนังในบริเวณนั้น ผู้ป่วยอาจเกาจนเป็นหนอง เมื่อพยาธิเดินทางผ่านปอดใน 1-2 สัปดาห์ต่อมา อาจทำให้มีอาการหลอดลมหรือปอดอักเสบได้
แต่อาการที่พบบ่อย คือ จุกเสียดแน่นที่ยอดอก ปวดท้อง หรือท้องเดิน ถ้ามีจำนวนพยาธิมาก จะทำให้มีอาการซีด มึนงง หน้ามืด อ่อนเพลีย เบื่ออาหาร น้ำหนักลด และถ้าซีดมาก ๆ อาจทำให้มีอาการบวมหรือหัวใจวายได้
อาการมากน้อยขึ้นกับจำนวนพยาธิที่มีอยู่ในลำไส้ (อาการซีดจะเกิดขึ้นเมื่อมีพยาธิมากกว่า 100 ตัวขึ้นไป) อายุของพยาธิ ความต้านทาน และภาวะทางโภชนาการของผู้ป่วย
ภาวะแทรกซ้อน
ที่พบบ่อย คือ โลหิตจางจากการขาดธาตุเหล็ก เนื่องจากการเสียเลือดเรื้อรัง หากปล่อยปละละเลยจนมีภาวะโลหิตจางรุนแรง อาจทำให้เกิดภาวะหัวใจวายได้
อาจทำให้ขาดอาหาร โดยเฉพาะอย่างยิ่งขาดสารโปรตีน ซึ่งหากขาดโปรตีนรุนแรงอาจทำให้ท้องมาน (มีน้ำในท้อง) ได้
เด็กที่เป็นโรคพยาธิปากขอเรื้อรัง อาจทำให้ร่างกายเจริญเติบโตช้า และสติปัญญาพร่อง เนื่องจากการขาดโปรตีนและธาตุเหล็กเรื้อรัง
การวินิจฉัย
การวินิจฉัยที่แน่ชัด คือ การตรวจอุจจาระจะพบไข่พยาธิ บางรายแพทย์จะทำการตรวจเลือดดูภาวะโลหิตจาง
การรักษาโดยแพทย์
แพทย์จะให้การดูแลรักษา ดังนี้
ให้ยาฆ่าพยาธิ เช่น มีเบนดาโซล หรืออัลเบนดาโซล
ถ้าซีด ให้กินยาบำรุงโลหิตติดต่อกัน 4-6 เดือน
ในรายที่มีภาวะหัวใจวายจากภาวะโลหิตจางรุนแรง แพทย์จะรับตัวไว้ในโรงพยาบาล ให้เลือด และให้ยารักษาภาวะหัวใจวาย
