แผลแอฟทัส* เป็นสาเหตุของอาการแผลเปื่อยในปากที่พบได้บ่อยที่สุดในคนทั่วไป โรคนี้พบได้ในคนทุกวัย พบบ่อยในกลุ่มวัยรุ่นและวัยหนุ่มสาว พบในผู้หญิงมากกว่าผู้ชาย มักเป็น ๆ หาย ๆ เป็นประจำโดยไม่มีภาวะแทรกซ้อนร้ายแรง นอกจากสร้างความรำคาญ เมื่ออายุมากขึ้นจะเป็นห่างออกไปเรื่อย ๆ บางรายอาจหายขาดเมื่ออายุมาก
*มีชื่อเรียกอื่น เช่น canker sore, ulcerative stomatitis, recurrent aphthous stomatitis ชาวบ้านมักเรียกว่า แผลร้อนใน
สาเหตุ
ยังไม่ทราบแน่ชัด เชื่อว่าเกิดจากปัจจัยหลายอย่างร่วมกัน และอาจมีความสัมพันธ์กับระบบภูมิคุ้มกันของร่างกาย (เช่น ภูมิคุ้มกันต่ำ การแพ้สารบางอย่าง การเกิดปฏิกิริยาภูมิต้านตนเองหรือออโตอิมมูน)
พบว่าร้อยละ 40 ของผู้ป่วยที่มีอาการกำเริบบ่อย จะมีประวัติโรคนี้ในครอบครัว สันนิษฐานว่าส่วนหนึ่งอาจเกี่ยวข้องกับการถ่ายทอดทางกรรมพันธุ์ อีกส่วนหนึ่งอาจเกี่ยวข้องกับการมีปัจจัยด้านสิ่งแวดล้อมร่วมกัน (เช่น อาหาร สิ่งกระตุ้นให้โรคกำเริบ)
ผู้ป่วยที่เกิดแผลแอฟทัส อาจพบว่ามีสิ่งกระตุ้นให้อาการกำเริบ อาทิ
ความเครียด เช่น ขณะคร่ำเคร่งกับงาน หรืออ่านหนังสือสอบ อดนอน
การได้รับบาดเจ็บในช่องปาก เช่น เยื่อบุปากหรือลิ้นถูกกัดหรือถูกแปรงสีฟัน ฟันปลอม หรืออาหารแข็ง ๆ กระทบกระแทก
การมีประจำเดือน
การใช้ยาสีฟันหรือน้ำยาบ้วนปากที่เจือปนสารลดแรงตึงผิว (มีคุณสมบัติในการทำความสะอาดและเกิดฟอง) เช่น sodium lauryl sulfate, sodium lauroyl sarcosinate
การแพ้อาหาร เช่น แป้งข้าวสาลี ไข่ นมวัว เนยแข็ง กาแฟ โคล่า ช็อกโกแลต อาหารที่มีรสเค็ม รสเผ็ด ของเปรี้ยวหรือผลไม้ที่มีฤทธิ์เป็นกรด (เช่น ส้ม ส้มโอ สับปะรด สตรอเบอร์รี่ มะเขือเทศ เป็นต้น)
การแพ้แบคทีเรียบางชนิดที่มีอยู่ในช่องปาก
การใช้ยา เช่น แอสไพริน ยาต้านการอักเสบที่ไม่ใช่สตีรอยด์ ยารักษาโรคกระดูกพรุน-อะเลนโดรเนต (alendronate)
ภาวะขาดธาตุเหล็ก สังกะสี กรดโฟลิก หรือวิตามินบี 12
ภาวะภูมิคุ้มกันต่ำ เช่น โรคเอดส์ ภาวะเม็ดเลือดขาวต่ำ
การป่วยเป็นโรคบางชนิด เช่น โรคลำไส้อักเสบเรื้อรัง (inflammatory bowel disease/IBD)*
การเลิกบุหรี่ อาจกระตุ้นให้เกิดแผลแอฟทัสในบางรายที่เลิกบุหรี่ใหม่ ๆ
*โรคลำไส้อักเสบเรื้อรัง (inflammatory bowel disease/IBD) ซึ่งยังไม่ทราบสาเหตุที่แน่ชัด
โรคนี้พบว่ามีการอักเสบของเยื่อบุลำไส้ ซึ่งเกิดจากปฏิกิริยาภูมิต้านตนเองหรือออโตอิมมูน (autoimmune) ถ้ามีการอักเสบจำกัดอยู่เฉพาะในลำไส้ใหญ่ เรียกว่า "โรคลำไส้ใหญ่อักเสบเรื้อรัง (ulcerative colitis)" ถ้ามีการอักเสบที่จุดใดจุดหนึ่งหรือหลายจุดของทางเดินอาหารตลอดแนวตั้งแต่ปากจนถึงทวารหนัก เรียกว่า "โรคโครห์น (Crohn’s disease)"
ผู้ป่วยมักมีอาการกำเริบเป็นครั้งคราว เป็น ๆ หาย ๆ อย่างเรื้อรัง ด้วยอาการปวดท้อง ท้องเดิน อุจจาระมีเลือดหรือมูกปน และอาจมีอาการถ่ายเป็นเลือด (เนื่องจากมีแผลที่ลำไส้ ทำให้มีเลือดออก) ซีด (จากการเสียเลือด) มีไข้ เบื่ออาหาร น้ำหนักลดร่วมด้วย
อาการ
อาการที่สำคัญคือ มีแผลเจ็บในช่องปาก ซึ่งอาจพบที่ริมฝีปาก (ด้านใน) กระพุ้งแก้ม ลิ้น (ด้านข้าง/ด้านใต้) เพดานอ่อน ลิ้นไก่ ผนังคอหอย ทอนซิล หรือพื้นปาก (เนื้อเยื่อที่อยู่ใต้ลิ้นและเหงือก) ส่วนใหญ่จะมีเพียงแผลเดียว บางรายอาจมีหลายแผลเกิดขึ้นพร้อมกัน
ในรายที่มีแผลที่บริเวณผนังคอหอย/ทอนซิล จะมีอาการเจ็บคอเพียงจุดเดียวหรือข้างเดียว (ซึ่งผู้ป่วยสามารถระบุตำแหน่งได้) โดยเฉพาะอย่างยิ่งจะรู้สึกเจ็บมากเวลากลืนหรือพูด ทำให้กลืนหรือพูดลำบาก และอาจมีอาการปวดร้าวไปที่หูข้างเดียวกันเวลากลืน
อาการเจ็บแผลมักเกิดขึ้นอย่างฉับพลัน ซึ่งค่อย ๆ เจ็บมากขึ้นทุกวันในช่วง 3-4 วันแรก หลังจากนั้นจะทุเลาลงไปเอง
